Students click on the Feedback icon in the course to access the activity and then must click the button Answer the questions. (Note: the magnifying glass icon is only for previewing, not for submitting responses.) Teacher view Teachers see the Feedback settings tabs as well as an Analysis tab and Show responses tab.

Jun 20, 2019 · As an adminstrator, go to Administration>Site Administration>Plugins>Activity Modules>Feedback. Change "Allow full anonymous" to "yes" Note that this only works for the front page, and NOT for courses with guest access. You will also need to check that Authenticated User and Guest role both have permission to Submit Feedback Activity.

This does happen sometimes. Usually, logging out of the course and then logging back in will take care of this problem.

Apart from the obvious use here of evaluating a course, feedback surveys have a variety of uses, both within a course and for non-logged in users on the front page. The ability to set them as "anonymous" means also that sensitive issues can be surveyed without students worrying who might see their responses. Some ideas:

There could be some parts of a Moodle distribution that are default set to collect feedback. The pages that are for administrators, teacher editors should be of interest to Moodle developers. Help pages should collect to the documenters. It should be possible to turn off these default settings if a site doesn't want to participate.
This survey is based on a well-known survey from Stephen Brookfield. By asking students to reflect on critical moments that they remember from the course you will (all) learn a lot about what is working and what is not.
