There are 2 ways to continue: either be fast and replace the portals with your own as the non-jumping player passes through them, or place 2 portals close to the previous ones so that the jumping player can reach all 4 without touching the floor in between. If done right, the achievement for doing that should unlock:

six different coursesThe co-op campaign is set after the events of Portal 2's single player campaign. In it, the two testing androids ATLAS and P-body of the Cooperative Testing Initiative carry out a series of tests in six different courses, each consisting of about eight or nine chambers.
The single-player game can take six to 10 hours to play, and the co-op campaign is at least five hours long.
Confirmed: You can beat coop by yourself (spoilers)

Portal 2 is in all aspects a bigger fuller and more comprehensive game with more variety, diversity, characters, laughs, gameplay mechanics, multiplayer and level editor. But nothing will ever beat the simplicity, originality of Portal 1.
Finally, here's the cold hard truth: Valve doesn't need to make Portal 3 because the fans are keeping the franchise alive without them.
One of the great joys of Portal 2 was the co-op campaign. It was smartly constructed, fun to play, and had more than its share of delightful moments, making good use of the extra complexities that a two person, four-portal world enabled.
"The game's two-player co-operative mode features its own entirely separate campaign with a unique story, test chambers, and two new player characters," it said. "This new mode forces players to reconsider everything they thought they knew about portals.
Nope. The two campaigns are independent of each other, with different characters, different 'storylines' (although the storyline in co-op doesn't show through so much) and different endings.
Portal 2 has usually larger builds with the detailing making it look interesting and not boring. Puzzles are composed of more complex logic without too many objectives. Thanks, Gemarakup.
It was released on April 19, 2011, for PC, PlayStation 3 and Xbox 360. Portal 2 is a sequel to Portal as well as the first game created by Valve to be rated E10+, compared to the M rating of all of their previous titles excluding the first Portal game and the original Day of Defeat, which were rated T.
Portal 2's main selling point is portals. Armed with your trusty Aperture Science Handheld Portal Device your mission is to solve a variety of puzzles using portals. Portals can be used to move objects, move yourself and even hurl you at great speeds in order to solve puzzles.
Genetic Lifeform and Disk Operating SystemGLaDOS (Genetic Lifeform and Disk Operating System) is a fictional artificially superintelligent computer system from the video game series Portal.
At the beginning of Portal 2, she is woken up for the first time after 50 days of sleep after the events of the Lab Rat comic, for a mandatory set of physical and mental exercises.
about 8½ HoursWhen focusing on the main objectives, Portal 2 is about 8½ Hours in length. If you're a gamer that strives to see all aspects of the game, you are likely to spend around 21 Hours to obtain 100% completion.

Now for using all the portals without touching the ground. One player opens a portal on the ground and puts the 2nd one close to it. Jump into it from some height to gain momentum. There are 2 ways to continue: either be fast and replace the portals with your own as the non-jumping player passes through them, or place 2 portals close to the previous ones so that the jumping player can reach all 4 without touching the floor in between. If done right, the achievement for doing that should unlock:
Drop your co-op partner in goo while they are gesturing by removing the bridge under them
Solve 3 co-op chambers in the Mass and Velocity course in under 60 seconds each
Having a second profile for the "Professor Portal" achievement is also HIGHLY recommended, but actually isn't needed . First off the game starts in a little video sequence where you see both players being attacked (yeah, you don't play as humans).
Now Professor Portal requires you to play Calibration Course online with a friend who hasn't played before. That means they have to have never played co-op AND have to be on your friends list. They cannot have completed Calibration Course or have been in any chambers. It must be done online, split-screen doesn't count. Also, you must have beaten the coop and been awarded You Saved Science. If all of those requirements have been met congrats:
For the second part, both players simply need to hit the buttons at the same time, which will allow the ball to drop and lead to the exit. 1-3.
While Portal 2's single player offered a hardy challenge, the real mind benders are found in the co-op campaign. The possibilities are much bigger withfour portals, and it can befrustrating whentwo brains can't crack the puzzle. While we don't recommend using the guide for the whole thing, working your noggin is kind of the whole point, getting frustrated at a particular puzzle happens to the best of us.
