It has been used successfully for more than 40 years to treat opioid use disorder and must be dispensed through specialized opioid treatment programs. Buprenorphine is a partial opioid agonist, meaning that it binds to those same opioid receptors but activates them less strongly than full agonists do.
They're Both Opioids Both methadone and buprenorphine are opioids. Buprenorphine is a semi-synthetic compound, being made from both natural and synthetic compounds, while methadone is a fully-synthetic compound. Both prevent withdrawal symptoms by interacting with opioid receptors in the brain.
The ASAM Treatment of Opioid Use Disorder Course provides eight hours of required education to qualify for the waiver to prescribe buprenorphine in an office-based setting and covers essential information for providers to identify, assess, diagnose, and manage patients with opioid use disorder (OUD). The course covers the latest information on medications and treatments for OUD. ASAM is an approved provider by CSAT/SAMHSA of DATA 2000 training.
The Moving Beyond the Barriers course helps waivered providers address barriers to implementing opioid use disorder treatment in their practices.
Practitioners who have had a waiver to treat 30 patients for at least one year may apply to increase their patient limit to 100. After one year at the 100-patient limit, physicians and qualifying other practitioners who meet certain criteria can apply to increase their patient limit to 275. Click here to access the application for a higher patient limit. SAMHSA will review applications within 45 days of receipt and will notify the applicant if they have been approved. DO NOT begin prescribing to the higher limit until you are approved.
The SAMHSA 275 Annual Report Form details all of the requirements necessary to complete and submit the form. This form may not display if SAMHSA does not determine the enrollee eligible for an increase to 275 patients.
For nurse practitioners (NPs), physician assistants (PAs), certified nurse certified nurse specialists (CNSs), certified registered nurse anesthetist (CRNAs), and certified nurse midwives (CNMs) to be eligible to apply for a buprenorphine waiver, they must complete 24 hours of training that covers the following topics: opioid maintenance and detoxification; clinical use of all FDA-approved drugs for medication-assisted treatment; patient assessment; treatment planning; psychosocial services; staff roles; and diversion control. Once a practitioner has taken the required training course and complied with other requirements listed below, they may apply for a waiver to begin treating 30 patients with buprenorphine. C ertain practitioners may immediately begin treating up to 100 patients if they provide medications for the treatment of opioid use disorder in a qualified practice setting. It is important to note that CNSs, CRNAs, and CNMs are able to prescribe buprenorphine to treat OUD until October 1, 2023.

Physicians who plan to apply for a patient increase to 275 must follow several requirements and standards. The MAT waiver training available from ASAM and PCSS are approved for 24 hours of Category I CME by the AAPA. Students cannot submit the training for CME credit retroactively.
The content of the MAT waiver training, both the 8-hour and 16-hour modules, is specified in law.
If the waiver training is made a required assignment, students who do not complete the training should be subject to whatever the program policy is on non-completion of program requirements.
Having students complete the training early in the clinical year may have the advantage of proximity to didactic learning and give students an opportunity to practice some of the skills they will develop from the training during their SCPEs.
Other programs are including all 24-hours online during the clinical year.
Programs are taking different approaches to incorporating the training into their curriculum. A number of programs are having students complete the 8-hour training component as part of a behavioral health or pharmacology course, with the remainder completed during the clinical year.
