Madara Uchiha met his demise in "Naruto Shippuden" episode 474. In the said chapter, Hashirama Senju proposed a treaty to Madara Uchiha which allegedly caused his death, after losing the tailed beast and evil statue.
First, when he was fighting the first Hokage, Hashirama killed him. The 2nd time, he died after entrusting everything in Obito's hand, he died a natural death. And the 3rd time, after Kaguya was sealed and all the Tailed Beast was released, Madara died for the 3rd and last time.
Naruto, Sasuke, Sakura and Kakashi manage to seal Kaguya, who reverts into ten tails before being sealed spitting out Madara. Eventually, Madara dies because of the toll of both the tailed beasts and Demonic Statue removed from his body.

Before that point it was thought that one needed their eyes to use Susanoo but not when papa Madara sliced logic to pieces and summoned it with no eyeballs. Sasuke went blind and lost his immediately. Shisui should have been able to use it with one eye or no eye as well.
Tobirama is the genius who created the Edo Tensei and doesn't know the release jutsu. Of course Madara conveniently knows how to release himself from Edo Tensei although he never was an Edo before. Like in what other situation would this plot jutsu be useful lol. Who predicted that they would become an Edo Tensei and created a release? And a release that won't even benefit you unless the person chooses to give you control of your body and personality.

While on a mission, Rin was captured and turned into a jinchuriki. Rin decided to sacrifice herself for the sake of the village. Her actions would inspire Kakashi to place great value on the lives of his friends. Her death would also cause Obito to seek revenge.

Sasuke's brother, Itachi Uchiha, was just as great a shinobi as Sasuke is today, albeit not nearly as powerful. While Itachi Uchiha was strong in his own right, he certainly wasn't even close to Madara Uchiha's level. With the Six Paths Powers at his disposal, there's absolutely no way for Madara to lose to Itachi, no matter what.
Consequently, there was a drastic difference between his and Madara Uchiha's powers as well. Although strong, Tobirama was certainly not powerful enough to defeat Madara Uchiha.

Using it as a catalyst, Madara used Hashirama's cells in order to extend his own lifespan. Once disconnected from the Mazo, Madara died instantly. Madara Uchiha didn't survive. The one who everybody thinks is Madara is acually Obito Uchiha because Obito said that he was going to take on Madara's legacy.
The Rinnegan allowed him to break the seal placed by the Sage of the Six Paths, and summon the Juubi's shell (The Gedo Mazo) from the moon.
Madara was able to obtain the Rinnegan only after infusing himself with Hashirama's DNA. This would be the first time in history that a Rinnegan would appear outside of Kaguya Ōtsutsuki’s direct blood line. The only downfall to Madara’s Rinnegan was that he didn’t have full access to the original’s abilities.
Madara used the Demonic Statue of the Outer Path to harvest Hashirama's DNA and create humanoids from it, bringing the Zetsu to life. Madara assumed that these Zetsus were created by him, but it would later be revealed that all the Zetsus were actually humans who had been merged with the God Tree.
