In the CPT manual, when a code has a full description, not requiring the use of indented descriptors, these codes are called: stand-alone codes.
When the code for a biopsy states "with biopsy, single or multiple," report the code once, regardless of the number of biopsies taken. When a biopsy is followed by excision of a different lesion, assign a code for both procedures. If a biopsy is reported with a lesion excision code, modify the biopsy code with -59.

When two (2) surgeons perform distinctly separate parts of the same procedure on the same day during the same operative session, modifier -62 should be used. Modifier -59 is used to report that the decision for surgery was made at that visit. The -RC modifier should be used with codes 92920-92929.
